2021-22 After School Application/RegistrationForm

College Heights After School Program


Due to the limited number of spots available for the 2021-22 school year, open spots will be filled through a lottery process to be held on April 1st, 2021.


To be considered for the ASP lottery all new families must complete theAfter School Program (ASP) Application/Registration Form and return it to the school by March 31st. (Please download, print, and complete the form.)


Families may drop off, email ( or mail (attn: Elecia Fulton) the completed application. If awarded a spot through the lottery you will  be expected to complete the financial agreement and submit the registration fee within 72 hours of notification.


The ASP operates between 2:50 pm - 6:00 p.m. while following the City Schools of Decatur school calendar.  In order to be eligible for the ASP,  students must be enrolled in the College Heights pre-k program.  More information about our ASP can be found on our website under Teaching and Learning/After School Program.
