If you have a concern about yourself or someone else, please complete the General K-12 Counseling Referral Form. Students, Parents/Guardians, Teachers, Administrators, etc. may use this form to refer students to their counselor. CSD Counselors and Student Support Services personnel will use the Student Support Tiered Service Model to address student needs.
