School Health Requirements
Complete the Annual Data Update through the Infinite Campus parent portal, including the health information section, to ensure we have the most current information.

Care Plans/Action Plans: To ensure we can best support your child's health needs in the upcoming school year, we require updated Health Forms for all students. If your child has a medical condition (e.g., asthma, seizures, diabetes, food allergies), a new healthcare plan, signed and dated by you and their physician, is required at the start of the new school year. See the list of health forms at the top of the page.

Medication at School: An Authorization of Medication Administration Form is required for each medication administered. Parents or guardians must drop off all medications at the clinic. Over-the-counter medications must be unopened (parent signature required, prescriber signature required if dosage exceeds guidelines or for regular use). Prescription medications require a prescriber's signature.

Self-Carry Authorization: For students authorized to self-carry emergency medications (such as inhalers, epinephrine auto-injectors, or insulin/diabetic supplies), the physician must indicate this on both the Care Plan and the Authorization for Student to Carry Medication form, confirming the student's ability to self-administer. This form requires signatures from the physician, student, and parent.

Immunizations: Georgia state law requires that all students have current immunization documentation on file to attend school. Students who do not have current immunization documentation on file may be excluded from school attendance until compliance is met in accordance with Georgia's immunization schedule and school requirements. Please take the necessary steps to ensure your student receives vaccines appropriate for their age group over the summer. This step will help the school health team prepare for the annual immunization audit conducted by DeKalb Public Health in the fall. School Health Clinic Visits: Students exhibiting symptoms of illness will be referred to the school clinic for evaluation and care. All symptomatic students will be sent home from school. We kindly ask that you arrange for your child picked up within one hour of being called.
