IMPORTANT INFORMATION – Residency Verification 

All 3rd graders are required to show proof of residency. Please present these documents before September 6th, 2019 to the front office of Talley Upper Elementary Street (ATTN: Yvett Battles).

One document is required from Section 1 AND Section 2 shown below:

Please note: all documents must show the name of parent/guardian.

Section 1 (one item from either “i” or “ii”)

1. Proof of homeownership
1. Warranty deed, quit-claim deed or security deed
2. Mortgage statement or payment coupon
3. Property tax statement
4. Settlement statement (HUD-1)
2. Proof of leased address
1. Lease (all leases are subject to verification from landlord)
1. Lease period must be current (not expired)
2. Lease must have an end date
3. Lease must be signed and dated by tenant and landlord
4. Student’s name must appear on lease as occupant
5. Lease must have name, address and phone number of landlord

Section 2

1. Utility Bill (current: within the last 30 days)
1. Power, water, or gas (must state address of the legal residence)
