    Registration Process

    Step One: Complete the CSD online registration form. (If you see a login page, please close the browser and re-attempt link.)

    Registration days and hours are Mondays, Wednesdays, and Fridays from 8:30 am - 11:30 am (excluding school breaks and holidays).

    Step Two
    : Gather your registration supporting documentation and go to 125 Electric Avenue to complete your registration process. Additional information about these documents is available on the Registration Supporting Documentation page.

    • Proof of Residency (lease, deed, payment/coupon book, property tax or settlement statement)
      • If providing a lease, please ensure all occupants are listed on the lease and provide a lease verification found here.
    • A paper copy of current utility bill (water, gas or electric)
    • Birth Certificate (certified original)
    • Immunization (Form 3231)
    • Vision, Hearing, Dental, and Nutrition (Form 3300)
    • Social Security card
    • Picture ID of parent/guardian
    • Report card and/or school records for students in grades 1 - 12


    Please note: The Central Office hours for Registration and Enrollment during the summer months and school holidays are subject to change.


    Students currently attending a CSD school are automatically enrolled for the next school year and do not need to go through the registration process. Parents will be asked to verify residency when their student enters kindergarten, 3rd grade, 6th grade, and 9th grade.
