Required Immunization Information

Georgia requires that students receive immunizations for vaccine-preventable diseases before and while attending school to maintain the health and wellness of school-aged children and adolescents.
According to Georgia State Law (O.C.G.A 20-2-771) and DPH Rules and Regulations for Schools and Childcare Facilities, a valid immunization certificate, Form #3231, or a notarized Affidavit of Religious Exemption, Form 2208, is required for school attendance. A medical exemption on Form #3231 with a current expiration date should be noted.

City Schools of Decatur has been partnering with the DeKalb Board of Health and other local agencies and medical providers to bring COVID-19 vaccine events to Decatur. The vaccine events will be communicated through school and district newsletters and social media.
