Medications, including over the counter preparations, can be administered to students with parental permission (Medication Authorization Form). Prescription medications must be brought to school in the original container labeled with the student’s name, date, name of medication, name of prescribing physician, dosage, time(s) the medication is to be given, and name of pharmacy filling the prescription. OTC medications shall be sent to school in the original manufacturer’s container. The manufacturer’s label must include the name of medication (brand or generic), strength of medication, instructions for use, and name of student written legibly.
