Medications, including over the counter preparations, can only be administered to students with a completed Authorization for Medication Administration form. This form was updated for the 2020-21 school year. The previous form will no longer be accepted. Prescription medications must be brought to school in the original container labeled with the student’s name, date, name of medication, name
of prescribing physician, dosage, time(s) the medication is to be given, and name of pharmacy filling the prescription. OTC medications must be sent to school in the original unopened manufacturer’s container. The manufacturer’s label must include name of medication (brand or generic), strength of medication, instructions for use, and name of student written legibly.
