School Clinic Overview

Check-Out Policy: The clinic is available for illnesses or injuries that occur during school hours. Students must have a valid pass before coming to the clinic. If a student is too ill to remain at school, a parent/guardian will be contacted to pick his/her child up. The clinic must get parent/guardian permission via email and telephone before releasing any student to anyone who is not on the approved pick-up list.
Immunizations: Any student who turns 16 must have an updated 3231 Immunization Form on file showing that they received their second dose of the MCV vaccine. This should be done in the month after the student turns 16.
Health Management Plans: If it has been indicated that a student has a current medical problem, a completed health management plan should be on file in the clinic. These forms help provide the medical information needed to best care for your child while at school and they must be updated each school year. These forms are available in the clinic or at your physicians office (ex: diabetes medical management plan, asthma action plan, allergy action plan or seizure action plan).
Medications: Medications that are medically necessary to be given during the school day:- The medication must be provided by a parent/guardian.
- An Administration of Medication Request form must be filled out by a parent/guardian and on file in the clinic. The form is available in the clinic or can be downloaded from the CSD website.
- All prescription medications must be in the original container with the students name and directions for administration printed on the pharmacy label.
All medications must be in the original container and stored in the clinic. Medication in envelopes and baggies will not be accepted. All medication must be brought into the clinic by a parent/guardian. No expired medications will be accepted.
Inhalers: Students carrying prescribed inhalers throughout the day must have an Asthma Action plan signed by a parent/guardian and a physician, on file in the clinic. The form is available in the clinic or can be downloaded from the CSD website.
